Purpose of the Job:

The overall purpose of the RN Consultant is to develop, guide and advise Assisted Living Managers or Assisted Living Supervisors, DON's, PACE Managers and their staff members in providing the best possible service and care to the residents within the Assisted Living Communities, Longterm Care Communities and PACE Centers. The primary focus of this RN Consultant will be the Longterm Care Communities but may also develop, guide and advise nursing staff managers at Immanuel Pathways PACE and the Assisted Living Communities. The primary responsibilities of this job are to ensure policies and procedures are compliant with state regulations while being understandable and adaptable in each community and center. Assists the nursing staff members and Executive Directors or Center Directors in the areas of compliance, change management, process improvement, staff education and training.  The incumbent performs at the highest level of customer service and ethics in dealing with residents, participants and staff. and Advising

  • Consults with AL Managers and AL Supervisors, Lead LPN, PACE nursing managers and staff, and Executive Director or Center Director in the areas of care management, problem resolution, change implementation, and compliance with regulations for assisted living and/or PACE/Pathways skilled care.
  • Organizes and conducts on-site audits as assigned for assisted living communities and/or PACE Pathways to assess and improve the quality of care delivered and compliance with regulations and Immanuel policies.
  • Offers recommendations to improve or maintain quality audit scores. Conducts competence assessment and mock surveys.
  • Advises and serves as a clinical resource for Immanuel nurses and health care services. Approves and advises on the assessment, re-assessment, care management and delegation of resident care in working with the Assisted Living Manager, Assisted Living Supervisor or Lead LPN.
  • Reviews information and determines if a resident is appropriate for move in and/or returning to the assisted living community, and supports care management as needed (in communities with an Assisted Living Supervisor and/or the Lead LPN).
  • Reviews standards of staffing and scheduling including Medication Aides-40 in assisted living, and Day Center Aides (C.N.A.) in PACE Pathways to ensure sufficiently qualified and competent staff members are available to provide care and services.
  • Takes calls for Assisted Living Communities and provides direction for Medication Aides-40 in resident care based on the Personalized Service Plan.
  • Provides direction and monitoring for Medication Aides-40 along with an Assisted Living Supervisor in those communities where appropriate.
  • Assists the nursing managers, supervisors and staff members in accuracy of order, timeliness or delivery and compliance related to contract and regulations for contracted services such as pharmacy and lab.
  • Reviews resident and/or participant incidents, accidents, emergencies, or instances of unusual behaviors to ensure that appropriate action is taken.
  • Attends and participates in meetings with nursing team and Executive Director/Center Director at the request of a nurse manager or supervisor.
  • Participates in budgeting and resource allocations to support quality and safety in operations.
  • Promotes operational readiness through staff development, leadership, consensus on operations/processes and decision making.
  • Applies general principles and modifies standard techniques to solve complex and difficult tasks that are reviewed for compliance with established criteria.
  • Incorporates consideration for quality and safety, with a focus on cost savings, risk reduction, improving efficiency and effective stewardship of resources when making decisions.
  • Promotes maximal productivity and efficiency of operations such as faster response time, fewer errors, effective and timely communication and optimal use of assets.
  • Supports reduction of risk and related costs to include actual and indirect cost such as loss of credibility or reputation with consumers or professional partners.


  • Develops, revises, and implements policies, procedures and associated documentation to ensure compliance with regulations for assisted living and/or PACE Pathways as assigned.
  • Instrumental in setting high standards for quality care as well as a safe and clean environment.
  • Develops procedures for proper equipment use, infection control, and disaster planning to comply with regulatory guidelines.
  • Oversees the quality of pharmacy, lab, podiatry, and other contracted services.
  • Follows up on incident reports to ensure that appropriate procedures and follow-through were done effectively and included reporting to the Department of Health and Human Services and Centers for Medicaid and Medicare Services, as appropriate.

Training and Guidance

  • Coordinates monthly educational programs that meet state requirements for health care staff members and employees who work with assisted living residents.  The programs may be delivered in person or on-line.
  • Coordinates and/or supports the development and implementation of the annual Competence Assessment training.
  • Substitutes for the Assisted Living Manager or Assisted Living Supervisor in the community when needed, and serves as on-call staff for emergency situations 24/7 as assigned.
  • Provides navigation guidance and care management for seniors and their families in the senior’s continuum of care.
  • Coordinates testing of competencies for health care staff members.


  • Supports the AL nurse in Local Council and Central Council work.
  • Attends continuing education workshops and classes to maintain license and educational requirements.
  • Embraces educational opportunities and/or professional coaching to enhance/improve consulting skills.
  • Works with outside vendors to build relationships that are mutually beneficial.
  • Participates and networks with professional organizations to promote quality care for seniors.
  • Performs related duties as required or requested.



  • Current and active RN license is required in the respective state in which he/she is employed.
  • Bachelor’s degree in nursing is preferred with a specialization in gerontology. Master’s degree in nursing is desirable.
  • Gerontological Nursing certification through the American Nurses Credentialing Center is preferred.
  • Health Care Administrator certification is desirable.
  • Assisted Living Administrator registry in the respective state in which he/she is employed is required when working in the communities; or willingness to obtain within the first year.
  • Equivalent years of experience may substitute for education requirement.


  • Five (5) years of general and related nursing experience is required. A specialization in assisted living, long term care, PACE, and/or geriatric nursing is desired.
  • One (1) year experience in a supervisory or management role.
  • Experience in the assisted living, long term care, and/or PACE environment is desired based on the working location of the RN Consultant.
  • Equivalent years of education may substitute for experience requirement.

Other Requirements –

  • Certification as a First Responder CPR/AED and First Aid by national standards or willingness to obtain within 60 days of hire.
  • Valid driver’s license.
