Overview

The role of the LPN is a support position within Immanuel. The LPN will provide support to the Assisted Living Manager and Assisted Living Supervisor to ensure the best possible service and care for its Assisted Living residents.  The primary responsibilities of this job includes carrying out the direction of the AL Manager or AL Supervisor, ensuring physician’s orders and pharmacy orders are clear and accurate, and providing resident needs as indicated.  Monitors health care tracking systems and audits performed in the department.  Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

Administrative Duties

·        Reviews processing of physicians’ orders for accuracy and completeness before signing off. If incomplete, ensures follow up is done with physician’s office until orders are complete.

·        Contacts physician’s office or receives calls from physician on behalf of the resident regarding appointments, health care concerns, requests, or other matters and acts as a liaison with resident.

·        Communicates with pharmacy regarding orders, delivery, errors or omissions.

·        Coordinates, or assists AL Manager/AL Supervisor/RN Consultant with move in or move out of residents. Communicates with hospital or skilled care/rehabilitation center in regard to admission or discharge planning for residents who must move to and from one of these facilities.

·        Ensures accuracy of Personalized Service Plans for residents.

Compliance

  • Monitors systems (MAR’s, AOD) for accuracy and runs monthly summaries
  • Completes monthly audits of AED, glucose monitors, and other systems.
  • Ensures medication counts, quantities and changeover is accurate and done in an organized manner.

Resident Care

  • Provides resident assistance in personal care, health maintenance and activities of daily living such as bathing, grooming, dressing, mobility and meal service assistance.
  • Administers medication as requested by the resident and prescribed by his/her physician and in accordance with statutes governing medication provision.
  • Maintains a safe and clean environment. Follows procedures for proper equipment use, infection control, disaster planning, and related policies and/or regulatory guidelines.

Other

  • Attends continuing education workshops and classes to maintain license requirements.
  • May assist AL Manager/AL Supervisor with tasks for the LPN’s development and growth.
  • Performs other duties as assigned or requested.

Qualifications

Education-

  • Current Licensed Practical Nurse (L.P.N.) license in the respective state in which he/she is employed is mandatory.

Experience-

  • Relevant experience as an aide (C.N.A. or Q.M.A.) or L.P.N. is required.
  • One (1) year of experience in an assisted living or skilled care community is preferred.
  • Equivalent years of education may substitute for experience requirement.

Other Requirements-

  • Certification in Basic Life Support and First Aid is preferred or willingness to get certified within 60 days of hire date.

KSA- Knowledge Skills and Abilities-

  • Knowledge of Immanuel, its mission and Promises, and how those are directly correlated to the incumbent’s job.
  • Knowledge of concepts, principles, and practices in the nursing field.
  • Knowledge of medical terminology, instruments and equipment and how to use and maintain them.
  • Knowledge of physical, mental, spiritual, and social needs of the senior and/or frail elderly population and their families

  • Skilled in establishing and maintaining effective working relationships with residents/participants, co-workers, family members, and vendors.
  • Ability to clearly communicate with physicians, their office personnel, pharmacies, and other service company representatives.

  • Ability to implement quality practices in personal behavior and job/department outcomes.
