Overview

Purpose of the Job:

The overall purpose to this job is to implement the health care components of the established service plan in the Center’s Clinic. The primary responsibilities of this job are to perform health assessments of participants as needed or on a regular basis to determine if and when their health conditions or individual needs have changed.  Provide information to the Interdisciplinary Team (IDT) to determine any revisions to their respective plan of care.  May represent the Clinic at IDT meetings.  Monitors participant’s response to medical treatment plan and nursing interventions, and revises plan of care as necessary. Reports and records results of the nursing assessment, care rendered, and the participant’s response to care. Performs nursing care in the clinic or triage for the participants. May act as lead in the clinic in the manager’s absence. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

75% Participant Care

  • Conducts an in-person comprehensive initial nursing assessment including problem identification and plan of care with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive care plan for each participant.
  • Conducts an in-person nursing re-assessment, semi-annually, and as needed.
  • Implements the health care components of the established service plan.
  • Performs health assessments of participants as needed or on a regular basis to determine if and when their health conditions or individual needs have changed. Provides information to the Interdisciplinary Team (IDT) to determine any revisions to their respective plan of care.
  • Monitors participant’s response to medical treatment plan and nursing interventions, and revises plan of care as necessary.
  • Reports and records results of the nursing assessment, care rendered, and the participant’s response to care.
  • Collaborates with other health care professionals and caregivers regarding provision of participant’s health care.
  • May represent the Clinic at Interdisciplinary Team (IDT) meetings. When in attendance participates in (IDT) meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery.
  • Reviews and maintains participant’s records, charts, and information pertinent to participant care. Documents in medical records according to Immanuel Pathways standards.
  • Administers prescribed medications and treatments in accordance with nursing standards and monitors medication compliance.
  • Assists in treatment, examination and testing of participants.
  • Assures certain health and personal care services are provided.
  • Educates participants, family and caregivers in regards to care, medications and treatments.
  • Provides skilled supportive and restorative nursing care to participants as ordered, re-evaluating their plan of care and health status on an on-going basis.
  • Provides health counseling to participants and their families on an individual or group basis as needed.
  • Maintains participants’ medical record and fulfills agency charting and reporting requirements.
  • Collaborates with other health care professionals and caregivers regarding provision of participant’s health care.

20% Compliance

  • In conjunction with the Clinic Manager, implements the Infection Control Plan to ensure a safe and sanitary environment. Responsible for tracking and maintaining post exposure evaluation and participant surveillance reports.
  • Follows all Immanuel Pathways policies and Procedures and OSHA safety guidelines.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, clients and families.
  • Participates in and supports Quality Improvement initiatives.
  • Maintains safe working environment. Follows Immanuel Pathways Safety policies and procedures.
  • Implements Exposure Control Plan
  • Complies with Emergency Preparedness Plan
  • Uses proper lifting and push/pull techniques, uses gait belt assistance on participant transfers as needed.
  • Provides first aid treatments as needed.
  • Acts within the scope of his or her authority to practice.

5% Other

  • Performs other duties as required or requested
  • Functions as the lead in the Clinic in manager’s absence
  • Participates in continuing education classes and any required staff and training meetings.
  • Maintains professional affiliations and any required certifications.

Qualifications

Education-

  • Bachelor’s degree in nursing preferred; professional nursing school graduate required.
  • Current License as a Registered Nurse (RN) in the respective state in which he/she is employed is required.
  • Cardio Pulmonary Resuscitation (CPR) certification required.

Experience-

  • Two (2) years of nursing experience in a hospital, nursing home or community-based setting- preferably in a geriatric care setting.
  • One (1) year experience working with the frail or elderly population is required.
  • Possess Job-specific competencies for Registered Nurse.

 

Other Requirements-

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).

KSA- Knowledge Skills and Abilities-.

  • Knowledge of physical, mental, and social needs of the frail elderly and their families.
  • Knowledge of local health care and geriatric service networks.
  • Knowledge of PC computer systems, including word processing, spreadsheet, calendar and database.
  • Knowledge of the various disciplines to assess the needs of the elderly population.
  • Skilled in written and oral communication.
  • Skilled in establishing and maintaining effective working relationships with physicians, participants, co-workers, and the public.
  • Skilled in making decisions and leadership implementing principles.
  • Skilled in managing and supervising managerial and professional staff members.
  • Skilled in facilitating group interaction, decisions and implementation processes.
  • Ability to effectively and efficiently plan, prioritize and follow-up on delegated responsibilities.
  • Ability to apply creative problem-solving skills to complex issues.
  • Ability to foster collaborative working relationships.
  • Ability to chart via Electronic Health Records
  • Proven experience and basic computer proficiency (internet, email, Microsoft Office)
