Gathers information  from  participants  and/or  their  family  members  about  health,  home environment and individual needs to share with In-Home Services RN, who then performs initial assessment, semi-annual reassessment, annual reassessment, and change of condition assessments of participants in the their respective home environments. Provides skilled supportive and restorative nursing care to participants as ordered, and educates participants in their  homes  about  health  care.  Assists  RN  or  Manager  with  the  coordination  of  each participant’s plan of care by initiating contact and services to be provided with appropriate community resources and Interdisciplinary Team (IDT). Maintains the participant’s medical record with thorough documentation. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

70% In-Home

Participant Care

  • Gathers information from participants and/or their family members about health, home environment and individual needs to share with In-Home Services RN in order to complete initial assessment and coordinate the plan of care with care-giving resources. May be asked to coordinate with the Interdisciplinary Team (IDT) to develop plan of care for each participant.
  • May attend IDT meeting as a representative of In-Home Services. Maintains regular attendance at, and participates in meetings, communicates participant changes, reports any plan of care decisions and coordination of 24-hour care delivery to the In-Home Services RN or Manager.
  • Gathers information about participants semi-annually at the request of the In-Home Services RN or Manager, including any changes of condition, and/or other health, home environment and individual needs. Provides information about assigned Immanuel Pathways participants to the RN to perform semiannual assessments.
  • Provides skilled supportive and restorative nursing care to participants as ordered, noting such care in their respective plan of care and health status to be reviewed and acted on by the RN.
  • Provides health counseling to participants and their families on an individual or group basis as needed.
  • Monitors medication compliance, administers medications and provides training in self-administration of all home meds as needed.
  • Maintains participants’ medical record and fulfills agency charting and reporting requirements.
  • Collaborates with other health care professionals and caregivers regarding provision of participant’s health care.
  • Reviews participant’s records, charts, and information pertinent to participant care.

25% Compliance

  • Follows all Immanuel Pathways policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Acts within the scope of his or her authority to practice.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Maintains safe working environment. Follows Immanuel Pathways Safety policies and procedures.
  • Implements Exposure Control Plan
  • Complies with Emergency Preparedness Plan.
  • Uses proper lifting and push/pull techniques, uses gait belt assistance on participant transfers as needed.

5% Other

  • Performs other duties as assigned or requested.
  • Participates in continuing education classes and any required staff and training meetings.
  • Maintains professional affiliations and any required certifications.






  • Current Licensed Practical Nurses License, in the respective state in which he/she is employed.


  • One (1) year of nursing experience in a hospital, nursing home or community based setting; home health experience preferred.
  • One (1) year of experience in working with the frail or elderly population required.

Other Requirements –

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).

KSA- Knowledge Skills and Abilities-

  • Knowledge of concepts, principles, and practices in the nursing field.


  • Knowledge of medical terminology, instruments and equipment and how to use and maintain them.


    • Knowledge of physical, mental, spiritual, and social needs of the senior and/or frail elderly population and their families

    • Skilled in establishing and maintaining effective working relationships with residents/participants, co-workers, family members, and vendors.
    • Ability to clearly communicate with physicians, their office personnel, pharmacies, and other service company representatives.
