    The overall purpose of this job is to be the liaison between therapy and nursing while promoting a restorative nursing philosophy among all community departments. The primary responsibilities of this job include the development, implementation, monitoring, and supervision of the restorative nursing program for the community. Supports and lives out Immanuel’s Mission and CHRIST Promises.

    Key Responsibilities and Duties of the Job

    Restorative Operations

    • Maintains a current census of resident’s participating in a restorative program on each neighborhood. For each neighborhood, these lists will be specific as to ambulation, range of motion, activities of daily living (ADL), assistive feeding devices, splints, hand cones/contracture care, bladder re-training, disease specific therapy or other restorative program details.
    • Works closely with the nurses and direct care Certified Nursing Assistants (CNA’s) to ensure the restorative plan of care is being followed thereby, promoting residents to their highest level of function.
    • Participates with Nursing Administration in development of restorative nursing policies and procedures.
    • Oversees that all restorative nursing measures are reflected on the care plan as an approach to the problem or need for which they are being done.
    • Oversees that residents get formalized therapy, as indicated, and that the necessary assistive feeding and/or ADL devices are obtained as recommended.
    • Works with Nursing and Dietary to see that assistive devices are consistently available and that they are addressed on the care plan.
    • Works closely with Therapy. Ensures that specific recommendations, when discharged from therapy are carried through by the restorative therapy team.
    • Performs or oversees functional assessments on admission and quarterly as indicated. Consults with nurses in making recommendations for restorative plan of care. Works with nurses in developing restorative plan of care.
    • Oversees incontinent management programs.
    • Attends care planning conferences and makes recommendations.
    • Sees that equipment and supplies are available and that they are in good working order.
    • Participates in Quality Improvement Program (QIP) audits for restorative nursing. Reviews monthly and quarterly
      quality indicator/measure lists and utilizes this information to problem solve.

    Staff Supervision & Training

    • Oversees and monitors the restorative therapy program and provides 1:1 teaching, as necessary.
    • Conducts routine classes for Restorative Nurse Tech (RNT) certificate courses.
    • Coordinates with Team Lead Education to orient new staff relative to roles, responsibilities, principles, and practices of restorative nursing.
    • Recruits, selects, trains, coaches/counsels, and supervises admissions staff.
    • Works in collaboration with Chief Nurse Executive setting goals and evaluating team members to produce desired outcomes.

    Compliance & Documentation

    • Sets up documentation flow sheets for daily documentation of restorative nursing measures for each resident on the program.
    • Makes a minimum of one assessment note per month – or more often if resident is not progressing. Reviews weekly notes made by RNT’s.
    • Tracks of progress in a formalized report and reports monthly at QAPI committee.
    • Completes assigned areas of the MDS, including Care Area Assessments.
    • Makes progressive/regression summary notes monthly that are specific to the restorative nursing measures being done.


    • Performs other duties as assigned or requested.




    • Current Registered Nurse (RN) license in the respective state in which he/she is employed is required.
    • Bachelor’s degree in nursing is preferred.


    • Three (3) years of hospital, long-term care, acute rehab or related experience is required.

    Other Requirements –

    • Certification in Basic Life Support and First Aid is preferred or willingness to get certified within 60 days of hire date.
    • Restorative/Rehabilitation certification preferred.

    KSA- Knowledge Skills and Abilities-

    • Knowledge of the MDS process and Care Plans.
    • Knowledge of EMR, specifically Point Click Care.
    • Knowledge of physical, mental, spiritual and social needs of the frail elderly and their families.
    • Knowledge of PC computer systems, including word processing, spreadsheet, calendar and database.
    • Knowledge of the various disciplines to assess the needs of the elderly population.
    • Strong knowledge of applicable laws, regulations, guidelines, and professional standards.
    • Skilled in making decisions and leadership implementing principles.
    • Skilled in facilitating group interaction, decisions and implementation processes.
    • Skills in effective written and verbal communication.
    • Skills in establishing and maintaining effective working relationships with residents, co-workers, family members, and vendors.
    • Ability to apply creative problem-solving skills to complex issues.
    • Ability to exhibit high standard of ethics, accountability and responsibility.
    • Ability to work well with diverse workforce.
    • Ability to provide timely, clear direction to staff.
    • Ability to ensure quality standards are met.
