Key Responsibilities and Duties of the Job

Participant Care Aide Management 85%

  • Ensure full compliance of all activities and operations performed within the Day Center and In-Home Aide Program are in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocols, licensing and certification requirements governing Immanuel Pathways operations.
  • Understand the goal of PACE is to keep participants at the lowest level of care feasible by leveraging our services to ensure participant safety and well-being.
  • Understand PACE regulations includes 24/7 care coordination and care delivery. 
  • Manage and prioritize staffing utilization based on actual participant needs and interdisciplinary team (IDT) authorizations.
  • Participate in the recruitment, hiring, and performance evaluation of Participant Care Aides and Day Center Supervisor, fostering a positive and cohesive team environment. 
  • Develop and provide training, conduct initial and annual competency assessments of staff, and evaluate the performance of all aides providing services.
  • Ensure that the role and responsibilities of all direct reports are defined and understood.
  • Provide leadership in the implementation of participant care aide engagement activities, ensuring a nurturing and stimulating environment for participant care aides.
  • Coordinate and schedule in-home and Day Center care services to meet the needs of program participants effectively and efficiently.
  • Manage scheduling, staffing budget, attendance and coverage for all Participant Care Aides.
  • Communicate scheduling changes and coverage to internal team and participants/caregivers.
  • Conduct initial and periodic in-home participant assessments to assess care needs and collaborate with the interdisciplinary team to update individualized care plans accordingly.
  • Maintain clinical competency and may be required to provide direct participant care.
  • Oversee and participate in state and/or federally regulated supervised aide visits are completed to ensure competency and customer service standards are met. 
  • Participate and collaborate as a voting member of interdisciplinary team to optimize the overall well-being and quality of care for program participants.
  • Collaborate with the interdisciplinary team and participant to develop a comprehensive, person-centered plan of care that promotes independence in the community, management of health conditions, and prevention of acute care admissions.
  • Understand how the role of participant care aid care directly affects the reduction of ER, hospital, SNF, and LTC use.
  • Act as a liaison between program participants, their families, and the PACE program administration, addressing concerns and ensuring effective communication. Report issues to leadership and use State and Federal regulations.
  • Oversee the documentation of participant care, observations, and incident reporting, ensuring accuracy, completeness, timeliness, and confidentiality per State and Federal regulations.
  • Participate in budget review and answers to any variances with overtime and agency use.
  • Participate in planning and provides justification for all staffing, equipment, and supply needs.
  • Participate in quality improvement activities including utilization management, clinical audits and work groups.
  • Participate in Center and Organizational leadership meetings.

Compliance 10%

  • Ensure compliance with all State and Federal program requirements, obtaining and maintaining certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal laws or rules.
  • Monitors Immanuel Pathways’ operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.
  • Safeguard the privacy and confidentiality of all company procedures, results, and information pertaining to employees, participants, clients, or families.
  • Adhere to Immanuel Pathways’ Safety policies and procedures.
  • Implement the Exposure Control Plan effectively.
  • Complies with Emergency Preparedness Plan.
  • Execute the Emergency Preparedness Plan in accordance with company protocols.

Other 5%

  • Actively participate in continuing education classes and mandatory  staff and training meetings.
  • Maintain professional affiliations and any required licensing or certifications.
  • Performs other duties as assigned or requested.

 

Skills & Requirements

Education-

  • Current Registered Nurses license, from the respective state in which he/she is employed is required.
  • Bachelor of Science Nursing (BSN) preferred; Graduate of a school of professional nursing.

Experience -

  • Two (2) years of nursing experience in a hospital, nursing home or community based Setting is preferred.
  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
  • Two (2) years of nurse management experience in a hospital, nursing home or community-based setting- preferably in a geriatric care setting required.
  • Equivalent years of education may substitute for experience requirement.

Other Requirements-

  • Active Home Health Licensed Administrator (depending on state) in good standing.
  • Be legally authorized (currently licensed or if applicable, certified, or registered) to practice the job’s functions and actions in the respective state in which he/she is employed.
  • Only act within the scope of his or her authority to practice in the respective state in which he/she is employed.
  • Agree to abide by the philosophy, practices, and protocols of the PACE organization.
  • Job specific competencies for the In-Home Services RN Supervisor will be met prior to assuming participant care.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED).
  • Reliable transportation is required as this position will require travel between locations.
